During the registration you should provide the following information:

• Full name;
• Address;
• PESEL (Polish nationals only);
• Type and number of your kayaking qualification certificate;
• Entry fee - indicate which: cash / bank transfer ;
• The competitions you intend to take part in;
• Team members (if you are registering a team);
• Date of arrival to the Championship;
• Agreement to having your personal data processed for the purpose of the Championship.
